Comprehensive in-home healthcare solutions.

Clinical Wellbeing Checkups

Weekly health checks completed by registered paramedics with any health concern being escalated to the team GP immediately, with an appropriate treatment plan being developed, all without the patient having to leave the comfort of their home.

Immunisations

Vaccinations for which the patient would otherwise have to leave their home to their medical centre can be avoided with Paracare, we complete onsite vaccinations in the comfort of the patient's home.

Medications can often become either too effective, or ineffective, our medication reviews ensure the prescribed medication and dose is actually working as it should, also being able to organise script refills, medication prescription (Antibiotics, pain relief, etc) through the team GP.

With a broader team of allied health staff, Paracare is able to deliver essential services such as podiatry, physiotherapy and dietitian, psychologist, occupational therapist to your door, so we can support you in maintaining your healthy lifestyle with convenience.

Post Hospital Discharge Monitoring

We often find that these patients experience a higher exposure to social isolation, leading to the deterioration of their mental health, therefore we advocate for and provide clients with non-clinical time, for a simple chat and catch up on life, ensuring they still have a means of human connection and personal care.

Post hospital discharge, patients are are increased risk of deterioration and reversion into their condition, therefore we offer daily check in services for the days following discharge, ensuring patients any deterioration is rapidly detected, and reintegration into their home is made easy.
